How do I make an appointment?

New Patients can call our office to schedule an appointment. When you arrive there are several forms that will need to be completed. You may also download the paper work from this website. If you do not complete the forms before your appointment please arrive 15 minutes early so may can have the paperwork done before you see your clinician.

Existing Patients may call the office or contact us through the patient portal to request an appointment and we will contact you.

What are your office hours?

Monday – Thursday: 8:00am – 7:00pm
Friday: 8:00am – 5:00pm
Closed for lunch from 12:00-1:00

What should I bring for my appointment?

Insurance card
Drivers License (If you have one)
All medications including supplements
Any Documents you downloaded and completed at home
A summary of your medical problems

How do I reschedule or cancel an appointment?

Please call us 24 hours prior to your appointment to reschedule. We may charge a fee if not given 24 hours notice.

What do I do if I need medical help after hours?

We offer urgent care to our established patients after hours through our on call service. If you have an urgent situation and call the main number after hours you will be prompted to connect with the answering service through a direct link. You may also call our answering service at (434) 972-1068. If you have a medical emergency please call 911 or go directly to the emergency room.

What insurance companies do you accept?

We participate with most major insurance companies. If you call ahead we can confirm your benefits including your responsibility for deductible, co-pay or co-insurance payments.

How do I get a written referral?

You will need to be seen by one of our primary care providers to receive a referral to specialist.

How do I get a refill of my medications?

Contact your pharmacy to see if you have any refills available on your prescription. If you do not, the fastest and easiest way to get your prescription refilled is by using the patient portal to make your refill request. You may also call the office. Because of the volume of requests we receive, we need to receive your request at least 2 days prior to your running out of medication. You will be called as soon as we have placed the prescription with your pharmacy.

How do I get my records copied or transferred?

You can fill out a release of records here in our office and your medical records can be transferred.

What are your billing procedures?

Because of the complexities of billing and collecting from third party payers, we utilize IMBS Insurance and Medical Billing Services, an independent billing company, located here in Charlottesville, for our patient financial services.

How do I make a suggestion, complaint or compliment?

Please feel free to contact our Practice Administrator with your praises or concerns. You may ask for him when you call the main number and he will get back to you as quickly as possible.
